We book an Initial Assessment Date asap. We are committed to enhancing the quality of life and well-being of every client we serve. Our dedicated staff passionately assist individuals with disabilities in setting realistic goals, utilizing a holistic, team-oriented approach to achieve them.

Ofcourse, We handle all necessary documentation for the Home Care Package Providers, or support coordinators and will create a formal service agreement that both parties must sign before we proceed. And happy to send our required documents to the Package Provider as per Aged Care Commission requirements.

For our home care package clients, we work collaboratively with all providers and offer our physiotherapy services at a reasonable cost under all four levels of packages. We adhere to the pricing guidelines established by the Aged Care Commission, charging the maximum allowable rate at present. We prioritize transparent communication regarding costs and verify funding availability over the phone or during your initial appointment to ensure seamless access to therapy within your current plan.

Our services primarily cater to aged care and home care package clients, focusing on enhancing their independence and well-being. Sessions are conducted at the participant's home, our clinic, or a community hydro pool to ensure convenience and accessibility. Additionally, assessments in the community may be arranged for activities outside the home.

Once we conduct an Initial assessment we complete a report and submit to support coordinator if required.

Thereafter, we conduct reassessment of your progress after one month and again after three months and so on using Standardized Outcome Measures. This ensures we stay on track to meet your desired goals and outcomes, which may vary.

This milestone tracking is outlined in your plan and is updated regularly.
We focus on specific goals with assessments and achievements that are tailored to these objectives.

All these is jotted down in report before your plan review meeting which will assist your support coordinator to secure funding or NDIS planner to fund your future plans accordingly.
